Online Form Improvements
From 12th October 2026, you may notice some additional questions when using our online forms to contact the practice about a health problem.
The questions are designed to help you provide useful information about your concern before a member of the practice team reviews your request.
How does it work?
Artificial intelligence (AI) uses the information you enter into the form to create up to five short follow-up questions. You will be told when AI is being used.
· The questions are optional and can be skipped.
· They will not appear for every request.
· The AI only uses information entered into your current form. It does not access your medical record.
· Your original request and any answers are reviewed by the practice team.
Does AI make decisions about my care?
No. It does not diagnose conditions, give treatment advice, decide how urgent your request is or make decisions about your care. Healthcare professionals remain responsible for reviewing your request and deciding what happens next.
Why are we introducing this?
The extra information may help us understand your needs sooner and reduce the need to contact you for further details.
Frequently asked questions
Do I have to answer the additional questions?
No. They are optional and you can submit your request without answering them.
Will everyone see them?
No. Additional questions are only shown when the system identifies that more information may be helpful.
Does the AI access my medical record?
No. It only uses information entered into your current online form.
Is information from my form used to train the AI?
Accurx, the company that provides our online forms, states that information submitted by patients through this service is not used to train the underlying AI model.
Can I still contact the practice in other ways?
Yes. If you are unable to complete the online forms, please telephone the surgery and we can complete them on your behalf. We kindly ask that you only call if you genuinely need assistance, so that we can keep our phone lines available for patients who are vulnerable or have additional needs and require support with completing their forms. Thank you for your understanding and cooperation.
